DIRECT Pathway

The Diagnostic and Interventional Radiology Enhanced Clinical Training (DIRECT) Pathway is a provisional pilot program with ABR oversight. It is ABR reviewed and approved.

Critical features:

  1. Program chairs or directors must submit applications for residents who want to enter the pathway. Residents cannot apply on their own.
  2. Residents entering the pathway need two years of clinical training. Residents may enter the new pathway from medical school or after two or more years of clinical training in other disciplines.
  3. The core diagnostic imaging training is 27 months.
  4. VIR  training includes nine months of nonfellowship training and 12 months of an ACGME-approved VIR fellowship, as described in the training sequence below.

Training sequence for residents entering PGY3 prior to 2010

Year Duration Training Exam
PGY
1 & 2
24 months Approved clinical training*
R1 12 months 10 diagnostic imaging + 2 VIR
R2 12 months 9 diagnostic imaging + 3 VIR
R3 12 months 12 VIR ACGME-approved fellowship
R4 12 months 8 diagnostic imaging + 4 VIR Oral exam in DR
12 months Clinical practice Subspecialty certification exam in VIR
*Clinical training in an ACGME-approved program. Clinical training might include a transitional year, plus another year in internal medicine, surgery, etc., or possibly in an integrated clinical year in diagnostic radiology. Most likely, two years of clinical training will be in another clinical discipline with entry directly into this program.

The initial certificate and subspecialty certificate (CAQ in VIR) issued by the ABR following accredited training under DIRECT and following successful completion of the examinations are the same certificates one can earn under the traditional training pathways. However, under DIRECT, one does not qualify to take the diagnostic radiology certifying examination until the R4 year is completed. An additional (practice) year is required before the subspecialty exam may be taken.

An additional nine months of clinical practice are required before the subspecialty exam may be taken.

DIRECT Pathway applications must come from program chairs or program directors. They are accepted three times per year: April 15, August 15, and December 15.
